Patient has bilateral hydronephrosis: dilation/distention of renal pelvis and calyces. Usually caused by urinary tract obstruction, other causes include retroperitoneal fibrosis and vesicoureteral reflux. This leads to backup of urine and increased pressure in the tubules -> increased tubular hydrostatic pressure. FA 2018 pg.483
