Urease hydrolyzes urea to release ammonia and CO2 which increases pH. Proteus, in particular, is predisposed to the creation of struvite (ammonium magnesium phosphate) stones.
Proteus mirabilis is uniquely motile and causes "swarming" on agar. On both X-ray and CT, struvite ammonium magnesium phosphate stones are radiopaque and look like a coffin-lid as crystals. They also commonly cause staghorn calculi.
