The question stem is asking what features led to the ineffectiveness of pharmacotherapy. Patient's presentation is consistent with Mycoplasma pneumoniae infection, which doesn't have a cell wall and therefore peptidoglycan, which is the site of action for penicillins. It does require sterols for growth, but that wouldn't explain why amoxicillin is ineffective.

jesuslovesyou  so does mycoplasma pneumo cause Bronchopneumo or Interstitial (atypical) pnemo. FA 683 (2020) lists Mycoplasma as "interstitial", if anyone could clarify why then are we saying this is mycoplasma? thanks +
calvin_and_hobbes  Mycoplasma pneumoniae is the classic organism associated with "cold" autoimmune hemolytic anemia. Q stem says "cold agglutinins is positive". IgM against oligosaccharide in Mycoplasma's membrane cross-reacts with self RBCs. That's why this is Mycoplasma. +1
