The answer is reassurance because this is all normal behavior. She's only snored TWICE in the past year w/ no daytime sleepiness or other problems.

It wouldn't be a sleep journal because it could imply that something is wrong, but there isn't. She's 27 and if anything she should upgrade from her husband complaining about nothing important.

A rule of thumb.

When seeing an option with "refer", it is almost always the WRONG answer in all NBMEs and Uworld questions.

The big take-away while reading this question for me was that, first-if you can tell that there is probably nothing really going wrong here with this patient then second-you should read the question carefully when it asks what the most appropriate "initial" action, and whenever you think a question is gearing towards a "behavioral" type question, pay close attention to comments the patient might make. The patient comes in because she is "concerned". Not because of symptoms. Bingo.
